Bumrungrad and Bangkok Hospital are genuinely world-class, and that is precisely the problem. The care is excellent, the bills are Western, and without cover a cancer course or major surgery runs to one or two million baht, £23,000 to £46,000, paid up front.

Public versus private

Public hospitals are functional and cheap, 500 to 2,000 THB a visit, but mean queues, limited English and shared wards. Few expats stay there by choice. Private care in Bangkok at Bumrungrad, Bangkok Hospital, Samitivej and BNH is internationally accredited, and priced to match:

  • GP consultation: 1,500–3,000 THB (£35–69)
  • Specialist consultation: 2,000–5,000 THB (£46–115)
  • Emergency-room visit: 5,000–15,000 THB (£115–345)
  • Surgery such as an appendectomy: 80,000–200,000 THB (£1,840–4,600)
  • ICU per day: 15,000–30,000 THB (£345–690)

The visa requirement

The O-A retirement visa requires health insurance with at least $100,000 of cover from an OIC-approved insurer. Most major providers qualify, but confirm approval before you pay, see our retirement visa guide for how this slots into the application.

Pre-existing conditions, and other fine print

Most insurers exclude or load pre-existing conditions. Declare everything, non-disclosure voids the policy when you can least afford it. Pacific Cross and AETNA tend to be more flexible than the international names.

  • Get quotes from at least three providers.
  • A higher excess cuts the premium sharply.
  • Ensure cover includes emergency evacuation to Bangkok.
  • Keep NHS entitlement live if you return to the UK often.
  • Do not skip dental, Thai dental work is superb value with cover.
